About the Role
Our client is seeking a highly organized and compassionate Remote Case Manager to join their dedicated social services team. This fully remote position allows you to provide essential support and guidance to individuals and families from the comfort of your home office. You will be responsible for assessing client needs, developing personalized service plans, connecting clients with vital community resources, and monitoring their progress towards achieving stability and self-sufficiency. This role requires exceptional communication skills, a strong understanding of social service landscapes, and the ability to manage a caseload effectively without direct, in-person supervision. You will be a critical link ensuring vulnerable individuals receive the support they need, regardless of their location.
Key Responsibilities Conduct comprehensive needs assessments with clients via phone, video conferencing, and secure messaging. Develop individualized case management plans tailored to each client's specific goals and challenges. Provide information, referrals, and direct assistance to connect clients with appropriate social services, including housing, employment, healthcare, and financial assistance. Monitor client progress regularly, documenting all interactions, assessments, and service plans in the case management system. Advocate for clients' needs and help them navigate complex systems and bureaucrongs. Maintain strict confidentiality and adhere to all agency policies and ethical standards. Collaborate effectively with internal team members and external service providers. Respond promptly to client inquiries and provide ongoing support and encouragement. Identify potential risks and develop mitigation strategies in client cases. Participate in virtual team meetings, training sessions, and case consultations. Requirements Bachelor's degree in Social Work, Psychology, Sociology, or a related field is required. Minimum of 3 years of experience in case management or social work, with demonstrated success in client support and resource linkage. Proven ability to manage a caseload effectively in a remote work environment. Excellent verbal and written communication skills, with strong active listening abilities. Proficiency with virtual communication tools (e.g., Zoom, Microsoft Teams) and case management software. Strong organizational skills and meticulous attention to detail for record-keeping. Ability to work independently with minimal supervision and maintain a high level of productivity. Demonstrated understanding of various social service needs (e.g., housing, mental health, employment). A stable internet connection and a dedicated, distraction-free workspace are essential.
